"Alexia's third mutation" is a cutscene in Resident Evil CODE:Veronica, playing near the end of the game at the Antarctic Base's control room passageway.

Plot

After receiving enough damage from Chris, Alexia starts going erratic while flapping her wings and the surrounding Ants going along with it. While going through her further mutations, the Linear Launcher was ready for use.
