A large mutated spider created from the T-Virus which appears in Resident Evil: Code Veronica and Resident Evil: Survivor 2 Code: Veronica. This spider was created as a B.O.W. using the battle data gathered from the Web Spinner. Like the Web Spinner, this spider can scale walls and ceilings, spit venom at it's prey and run at considerable speeds. It is, however, much more durable, taking more ammunition to kill. These spiders can be found on Rockfort Island and the Antarctic Research Facility where it appears they were being created. When the player attacks these spiders, the spinnerets will occasionally be blown off and the black widow will be able to move much faster and jump at the player repeatedly, greatly increasing this combat abilities; some fans refer to the form without spinnerets as an "attack form".

A larger version of this creature will appear in the Antarctic Research Facility. It is approximately the to the body of the size of Nosferatu's body, minus its long insect leg-like appendages, has led some to theorize that this creature actually detached itself from the body of Nosferatu.
