As a subspecies of Regenerador, Iron Maidens share largely the same features. What sets them apart, however, is the growth of sharp spikes on their bodies. As well, they seem to have lost their eyes, and when moving shall often twitch and convulse violently. These spikes are strong enough to kill a human opponent, should they be grabbed and forced to hug the Iron Maiden.[1][excerpt 1]

In the Resident Evil 4 remake, the Iron Maiden is shown to be an evolution of the Regenerador, rather than a subspecies. When a Regenerador has had all of its Plaga parasites destroyed, it will quickly alter its physiology in response and become the Iron Maiden. The Iron Maiden's skin becomes much paler and leathery than its Regenerador counterpart, with areas of muscle and flesh being exposed through gaps in response to severe damage. Its main distinct attribute, however, is the addition of a bioluminescent weak point exposed within its gaping maw.

Upon death, an Iron Maiden will convulse and writhe before exploding, after which its spikes will fire out toward any unfortunate individual.
