Lisa Trevor can be regarded as a biological melting pot of Umbrella research, having been used in experiments for over twenty years. Her superhuman strength, first recorded in 1967, stemmed from her initial infection with the Type-B Progenitor Virus strain, which otherwise would have killed her had she not been adaptive. Trevor developed a bone deformity where her arms had grown out of proportion to the rest of her body, which could though not conclusively be explained as over-production of Human Growth Hormone, a known side-effect in Progenitor strain diseases. In 1988, Trevor mutated after her immune system broke down a NE-α Type parasite. By 1998, a large teratoma had grown on her back where an eyeball had grown from, though there is no record of when this first appeared. Trevor's body also possessed heightened regenerative abilities due to the presence of the G-virus in her system, allowing her to survive a planned execution and escape despite being under at least seventy-two hours of observation. This ability rendered light weapons fire useless against her, with researchers familiar with her such as Dr. Albert Wesker considering it worthwhile to avoid her following her escape.

In terms of intelligence, Trevor displayed a number of human characteristics in spite of the brain damage she faced over the years. In 1998, she was observed living in a shed near the Spencer Mansion, having lit a fire to stay warm. She also managed to retain a crude ability to write out her thoughts as evidenced by several misspelled journal entries found by the Stars team throughout the mansion grounds. In spite of this, Trevor was prone to delusional thinking, believing her mother Jessica Trevor was still alive in the mansion grounds and was missing a face. This delusion led to several murders committed by Trevor, wherein the victims' faces were removed, which Trevor believed had been stolen from her mother and should be returned. Trevor's ability to speak was limited, and she was only seen during the 1998 incident to say the word "mother".
