The Examination room is located in Raccoon General Hospital. The player can access this room either from the locker room or from the Central waiting room.

Guide
The player can find the Card Key Level 1 on a zombie that is laying in a chair next to the x-ray photos. When the player enters this room a zombie will break through the nearby window and will attack the player should they get too close.
