The rats of the sewers were the original carriers of the t-virus that directly caused the Raccoon City Outbreak - as opposed to the leeches, which caused the outbreak in the Arklay Mountains.
Infected rats do not normally attack humans. The only known example of an attack was on an injured USS member seen in the introductory movie to Resident Evil: Outbreak.
